Reports help teams export grievance, complaint, and concern data for leadership review, QAPI, operational follow-up, and survey preparation.

Create a report

  1. Open Reports.
  2. Select New report.
  3. Choose the grievance log report.
  4. Select the format: CSV, XLSX, or PDF.
  5. Choose filters such as facility, status, category, assignee, or date range.
  6. Start the report.

Download a report

Reports may take a moment to generate. When the report is ready, select Download.

Include attachments

If available, choose the option to include attachments when you need the supporting case files in the export package.

Before sharing a report

Confirm the facility, date range, status filters, and included attachments. Reports may contain sensitive resident or reporter information, so share them only with approved team members.
