  4. Copy the link.
You can share the link with approved facility channels such as:
  • Resident or family handouts
  • Internal facility materials
  • A QR code
  • A facility website page
  • Admission or orientation packets

Review submitted concerns

Submissions create records for facility review. Review the intake details, confirm routing, and assign follow-up before working the case. Use clear language around the link, such as “Submit a concern” or “Share a grievance or complaint.” This helps residents and families recognize that concerns, complaints, and grievances can all be submitted through the same channel.
